We'll need to contact you at various times after you have left IBM and before you retire, so please make sure you tell us about any change to your address by completing this form and sending it in. Alternatively, you can simply write in, fax or send an email with the details.
On any communication, please remember to include your full name and employee serial number and if you're sending an email please include your National Insurance number and date of birth.
For your security, we can't accept notification of a change of address by telephone.
You should also update your Beneficiary Nomination if your personal circumstances change that may affect the payment of any benefits on death.
