Change of Freestyle investment instructions for DB ASCs/AVCs
Use this form to change the investment of any existing ASC/AVC fund you have from your Defined Benefit Plan membership.
Request to transfer-in. Ask the Trustee to consider the transfer of other pension benefits into the M Plan.
Beneficiary Nomination. Let the Trustee know your wishes about who should receive any lump sum payments in the event of your death.
Friends Provident and/or Equitable Life Disinvestment. Use this form to disinvest from the FP With profits policy and reinvest Legal & General. (NB: M Plan members won't have any investments with Equitable Life.)
Medical Application Form. Use this form if you are applying to purchase an annuity and believe you have a condition that will impair your life expectancy.
