M Plan - New Investment Instructions. Use this form to change how your Retirement Account is invested including:
- Lifestyle or Freestyle strategies
- between any of the L&G funds
- percentages in each fund
Change of Address. Notify us every time you move house. To be used in any plan, but not for current members.
Friends Provident and/or Equitable Life Disinvestment. Use this form to disinvest from the FP and/or EL With profits policies and reinvest in Legal & General. (NB: M Plan members won't have any investment with Equitable Life.)
Beneficiary Nomination. Let the Trustee know your wishes about who should receive any lump sum payments in the event of your death.
Medical Application Form. Use this form if you are applying to purchase an annuity and believe you have a condition that will impair your life expectancy.
