Friends Provident and/or Equitable Life Disinvestment. Use this form to disinvest from the Friends Provident and/or Equitable Life With profits policies and reinvest in BlackRock Advisors (UK) Limited or Legal & General.

Beneficiary Nomination. Let the Trustee know your wishes about who should receive any lump sum payments in the event of your death.
