Beneficiary Nomination. Let the Trustee know your wishes about who should receive any lump sum payments in the event of your death. Even if your nomination hasn't changed recently, completion of this form will give a clear indication to the Trustee who you would like to receive any payments.

Change of Bank Details. Keep us informed of any changes to your bank or building society account details.

Change of Address. Notify us every time you move house. To be used in any plan, but not for current members.
